Terms & Conditions
- The membership plan is open to adult patients.
- All patients must have had a full examination at Evolve dentistry prior to joining the Membership Plan.
- Payment is by monthly Direct Debit.
- If the Direct Debit is cancelled within 1 year then full treatment fees will need to be paid retrospectively for benefits already received.
- Once you have been a member of the plan for six months, paying by monthly Direct Debit, you will be entitled to free home tooth whitening.
- Home whitening trays are provided with 14 nights of gel. Annual top up gel will be provided (3 tubes) for the period of time membership is maintained. This treatment can be upgraded to an in-surgery treatment at a cost of £400. Home trays and annual top up is still provided.
- As an alternative to free home whitening for members who pay by monthly Direct Debit, free child dental care will be provided. This benefit extends to one child per parent. (Please talk to us if you have more than 2 children). This includes examinations, oral hygiene advice including fissure sealants, fillings and extractions on baby teeth (up to 2 per year). Extensive treatment or treatment required on adult teeth will be charged separately. Children covered by this benefit must be under 18 years of age and are only covered whilst membership of the parents is maintained.
- Once you have chosen the complementary benefit of tooth whitening or child dental care, they are not interchangeable.
- Benefits include a hygienist appointment every 6 months of Membership term.
- Terms and conditions are reviewed every 6 months and are subject to change.
To be able to offer these fantastic benefits, membership is strictly limited and benefits can be revised at any time.
- There is an initial £10 registration fee payable with the first direct debit.
- Free out of hours cover, overseas cover, hospital cover and accidental insurance starts after the first Direct Debit payment has been received by Evolve Dentistry.
Cancelling your Membership
If you find you ever wish to end the agreement you can do so simply by giving us one month’s notice. Naturally, if amounts due to us are not paid for two months in a row we may terminate the arrangement.
